The Association of Community Services of Howard County (ACS) is pleased to announce the CRISP eReferral tool is now available to ACS members. CRISP is a free application to help you receive more referrals from health care providers in your service area.
What is CRISP?
Chesapeake Regional Information System for our Patients (CRISP) is a non-profit organization that facilitates the electronic transfer of clinical information between disparate health information systems. As the regional health information exchange of Maryland since 2009, CRISP connects providers with technology to improve patient care and outcomes.
What is the eReferral Tool?
The CRISP eReferral tool is an application designed to allow health care providers and select members of their staff to refer patients to Community Based Organizations (CBO) across the region that provide an array of support programs and services that address a patient's social needs or address underlying causes of poor health outcomes such as food, housing or transportation insecurity (aka Social Determinants of Health or “SDOH”). The eReferral tool provides the following benefits:
Healthcare Providers: Most Maryland healthcare providers (doctors, hospitals, skilled nursing/long term care facilities, etc.) already use CRISP to get information about their patients’ encounters with other providers.
Community Based Organizations (ACS Members): The CRISP eReferral tool helps you build stronger referral channels with health care providers throughout your service area.
Consumers: Faster, more complete communication between referring healthcare providers and community-based organizations addressing social determinants of health means smoother transitions of care and better outcomes for those you serve.
